Conducted by Assured Information Security (AIS) or Other External Entities

I authorize AIS or any third party contracted by AIS or designated media organizations to photograph, interview and/or record me. I hereby grant full permission to AIS and any of its affiliates (including ORION) to use my image, recording, interview and/or written statement, and name in any educational publication or advertising/marketing materials (print or electronic). This consent also serves to waive all rights of privacy or compensation. 

I hereby release and agree to indemnify and hold harmless AIS, its affiliates and their directors, officers and employees from any injury and/or damages sustained as a result of such photographing, filming, videotaping, recording and/or interviewing, including but not limited to claims of personal injury, property damage, invasion of privacy and/or breach of confidentiality. 

I have the right to request that photographing, filming, videotaping, recording, interviewing and/or publication of my statement be stopped. I also have the right to rescind consent for use by making my request known in writing to the Director of Communications & Marketing. The use of photography, filming, videotaping, recording and/or interviewing done by news media is not under the control of AIS and I understand that once I provide consent to news media, I will not have a right to rescind unless the media agrees.
